Continuum Outpatient Center is a Texas-based mental health and substance abuse program for adults (18 and older) to come for Intensive Outpatient Programs (IOP) and Partial Hospitalization Programs (PHP) . Our curriculum is designed to help patients overcome past trauma, prevent relapse, heal in their relationships, and learn skills to enhance in their lives. Continuum Outpatient Center is a safe place for the community and creating new healthy habits. We provide an individualized approach with our masters level clinicians while utilizing holistic and evidence based treatment modalities - such as incorporating MAT and trauma-focused therapies. We value accountability, quality care, teamwork, integrity, and continuum improvement.
